2) Provide background details and reports if possible to ensure placement suitability.
3) We will provide assistance in the completion of the risk assessment and referral forms prior to placement.
4) Arrangements are made to ensure the child’s arrival at our service is fully planned and managed.
We believe it is essential for Social Workers and other professionals to visit Horizons to allow them a better understanding of the provision of care for the young people being referred - this will also give them the opportunity to meet the staff team.
We always have a senior manager available to discuss referrals and possible placements. Sharing of information is key to a successful referral as it allows the staff in Horizons to implement strategies to ensure a smooth transition for the young person. Background and current information will assist us in ensuring full risk assessments and appropriate action plans are created.
Once the placement is agreed, we will ask the referrer to complete an initial risk assessment and a referral form, the staff will be happy to assist in this if required. When complete, support and arrangements for the child's transition and arrival can be organised.
We provide close support to all our young people, offering a full range of holistic interventions to facilitate a sustained and meaningful placement.
We welcome referrals for young people who may be at risk of secure care or require intensive support to meet their complex needs. Each House has a homely environment creating a platform for life space interventions to support the most complex challenging behaviours.
We have the staffing structure in place at all times to offer emergency placements to children and young people in crisis, including same day placements. Emergency placements are subject to our house having a space at the time. Contact any of the Management Team using the contact details above to check our availability.
The placement cost will be discussed during the referral stages and the necessary paperwork completed prior to admission.
Our administration team will be able to assist with matters relating to balances due or outstanding.
In exceptional circumstances i.e., Emergency placements, we understand that the provision of care is paramount and the creation of active accounts will not delay a placement.
